Drug family: Antidepressant imipraminic
WHEN IS ELAVIL PRESCRIBED?
This drug is an antidepressant that belongs to the family of imipraminic antidepressants. It has atropine effects, which do not contribute to its activity, but which explain certain contraindications and side effects.

nocturnal enuresis in children (in the absence of lesions of the urinary tract).

CONTRAINDICATIONS OF THE DRUG ELAVIL
This medication should not be used in the following cases:
Risk of angle-closure glaucoma,

risk of urine retention (prostate adenoma...),

recent myocardial infarction (less than 3 months),

in association with non-selective MAOIs or with drugs containing sultopride.

CAUTION
This drug has atropine effects. It may cause acute glaucoma in susceptible individuals: red, hard, painful eye with blurred vision. An urgent consultation with an ophthalmologist is necessary.

All antidepressant treatments are likely to induce a manic episode in susceptible individuals: in case of euphoria, unusual excitement or abnormal behavior occurring under treatment, prompt medical consultation is necessary.

Precautions are required in case of epilepsy, heart disease, liver failure, renal failure and in the elderly, especially in case of chronic constipation, tendency to dizziness or low blood pressure.

Before a local anaesthesia, especially at the dentist's, report the use of this medication.

Avoid alcoholic beverages: increased risk of drowsiness.

Caution: driverIf you have to drive or operate dangerous machinery, make sure that this drug does not affect your alertness.

INTERACTIONS OF ELAVIL WITH OTHER SUBSTANCES
This drug should not be used in combination with non-selective MAOIs (MARSILID): specific time periods must be observed between stopping the MAOI and starting treatment with this drug, and vice versa.

It may also interact with A-selective MAOIs and with drugs containing any of the following: adrenaline, norepinephrine, clonidine, guanfacine.

Also tell your doctor or pharmacist if you are taking a serotonin reuptake inhibitor antidepressant, an anticonvulsant, an antihypertensive, an atropine, a sedative or a drug containing carbamazepine or baclofen.

FERTILITY, PREGNANCY AND LACTATION
PREGNANCY:
If pregnancy occurs during treatment, consult your physician. He/she will assess the possible risk of using this medication in your case.

If the treatment is continued until delivery, medical supervision of the newborn is necessary.

BREASTFEEDING :
This drug is likely to pass weakly into breast milk: breastfeeding is not recommended without medical advice.

DIRECTIONS FOR USE AND DOSAGE OF ELAVIL
The daily dose is usually taken in one sitting, usually in the evening.

The tablets can be swallowed either during or outside of meals.

USUAL DOSAGE :
Depression:

Adult: 75 mg to 150 mg per day, or 3 to 6 tablets per day.

Enuresis:

Child 6 to 10 years: 10 to 20 mg daily, at night at bedtime.

Children from 11 to 16 years old: 25 to 50 mg per day, i.e. 1 to 2 tablets in the evening at bedtime.

ADVICE
The effectiveness of antidepressants is generally not immediate. It can take up to several weeks for the beneficial effects to be felt. Possible side effects appearing at the beginning of treatment should not lead you to stop it prematurely.

In people suffering from depression, an anxious state may appear or worsen at the beginning of treatment; an anxiolytic is sometimes prescribed temporarily by the physician to prevent or treat these manifestations.

Once the symptoms of depression have disappeared, it is essential that the treatment be continued for the prescribed duration in order to consolidate the recovery. Prematurely stopping the treatment could lead to a relapse. The duration of an antidepressant treatment is rarely less than 6 months.

Disorders, sometimes of severe intensity, may appear when antidepressant treatment is stopped: anxiety, guilt, nightmares, dizziness, tremors. It is preferable to stop the treatment gradually by decreasing the dosage or by spacing out the doses to avoid these undesirable effects observed in particular in the event of abrupt interruption.

Weight gain may be observed during treatment. A balanced diet, and if necessary the advice of a nutritionist, can prevent and limit this phenomenon.

The use of this medication in cases of enuresis is only conceived within the framework of a more global treatment: re-education, possible psychological care.

POSSIBLE SIDE EFFECTS OF ELAVIL
Like all imipraminic antidepressants :
dry mouth, constipation, accommodation disorders, tachycardia, sweating, difficulty urinating, sexual disorders, orthostatic hypotension, somnolence;

more rarely: tremors, convulsions in susceptible individuals, confusion of ideas, weight gain, abnormal breast pressure, milk flow from the nipple, hot flashes, allergic reaction, hepatitis, abnormal blood count, heart problems, abnormal excitement.
